The Service

FormReceipt helps you verify that contact forms you configure remain functional by performing automated browser interactions, storing run artifacts (such as screenshots), sending confirmation workflows, and providing dashboards and alerts.

Accounts and eligibility

You must provide accurate registration information and keep credentials confidential. You are responsible for activity under your account.

Customer responsibilities and acceptable use

• You must only configure monitoring for websites, forms, and recipients you own or have explicit authorization to test.
• You must comply with applicable laws (including anti-spam and privacy laws) and respect third-party rights.
• You must not use the Service to probe, harass, attack, or overload systems you are not permitted to test, or to bypass security controls (including CAPTCHAs) where doing so would violate law or site policies.
• Automation requests are identifiable in server logs via our browser user agent (including identifiers such as FormReceiptWorker/1.0).

Third-party websites

Your configured pages may be operated by third parties. We do not control third-party sites and are not responsible for their availability, behavior, or policies.

Plans, billing, and taxes

Paid features may be offered through our billing partner. Fees, renewals, and cancellation are governed by the checkout terms presented at purchase and the billing provider’s policies where applicable.

Service changes and availability

We may modify or discontinue features. The Service depends on third-party infrastructure and target websites; downtime and failures may occur.

Disclaimer of warranties

THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, TO THE MAXIMUM EXTENT PERMITTED BY LAW.

Limitation of liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, NEITHER FORMRECEIPT NOR ITS SUPPLIERS WILL BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR EXEMPLARY DAMAGES, OR LOSS OF PROFITS, DATA, OR BUSINESS OPPORTUNITY. FORMRECEIPT’S AND ITS SUPPLIERS’ AGGREGATE LIABILITY FOR CLAIMS ARISING OUT OF THE SERVICE IS LIMITED TO THE GREATER OF (A) THE AMOUNTS YOU PAID FOR THE SERVICE IN THE 12 MONTHS BEFORE THE CLAIM OR (B) US $50.

Indemnity

You will defend and indemnify FormReceipt against claims arising from your misuse of the Service, your configuration of monitoring targets, or your violation of these Terms or law.

Termination

You may stop using the Service at any time. We may suspend or terminate access if you violate these Terms, create risk, or if we discontinue the Service. Provisions intended to survive will survive termination.

Governing law and disputes

These Terms are governed by the laws applicable to FormReceipt’s operating jurisdiction, without regard to conflict-of-law principles, except where prohibited by consumer protection rules that cannot be waived.
